Welcome New Employees and Affiliates

Welcome! We are excited to have you join us here at Headwaters Health Care Centre.  

Headwaters is a mid-sized community hospital located in Orangeville that is dedicated to providing excellent care to the residents of Dufferin County-Caledon. Accredited with Exemplary Standing by Accreditation Canada, Headwaters offers acute and complex continuing care with 24/7 Emergency Department coverage, extensive outpatient services in its Ambulatory Care Centre, and is also a proud partner in the Hills of Headwaters Collaborative Ontario Health Team.  As a team of over 700 staff and over 160 physicians, we are invested in the health of our community and are continually striving to live out our purpose of being One Community, Caring Together.

In order to ensure a successful transition to your new role, please complete the following steps by the deadlines indicated below:
 
  • Step 1: Submit the completed New Hire Documentation package to Human Resources no less than 7 days prior to your first day of employment.
  • Step 2: It is important to understand the terms and conditions of your employment with the Hospital. We would ask that you review the Policies and Procedures included in your Offer of Employment email.
  • Step 3: Complete the Health Surveillance form and submit to Occupational Health & Safety prior to your start date.
  • Step 4: Complete your General Orientation and assigned E-Learning modules (HR will provide you with the login information following receipt of your New Hire Documentation).
  • Step 5: Review the final checklist to stay on track.

Step 1: New Hire Documentation

Download this package and complete all forms contained within. You will also need to complete both the Federal Personal Tax Credit Form and Provincial Personal Tax Credit Form. 

All forms must be submitted to hr@headwatershealth.ca a minimum of seven (7) days prior to your start date, including a photo matching the specifications as per the New Hire Photo ID Requirements. This is to ensure that your computer access, Headwaters email, and ID badge are ready for you when you arrive on your first day. 

Important note: You will not be able to complete the Orientation modules (Step 4) until Human Resources has received and processed these documents.  Please ensure they are submitted on time to avoid delay.
 

Step 2: Review Policies and Procedures

In the email you received containing your Offer of Employment, there is an attachment with several policies that will need to be reviewed including the Privacy Policy, Media and Social Media Use, Confidentiality, and Employment Standards.  We ask that you review these prior to signing the Privacy and Security Forms and submit back with your New Hire Documentation a minimum of seven (7) days prior to your start date.

Step 3: Occupational Health Documentation

The Headwaters Healthcare Centre (HHCC) Communicable Disease Surveillance Policy adhere to the Communicable Disease Surveillance protocols published jointly by the Ontario Hospital Association, the Ontario Medical Association, and the Ministry of Health. Occupational Health and Safety and Infection Prevention & Control work collaboratively in the development of communicable disease protocols that protect both staff and patients.

Headwaters requires that all new employees participate in a health review completed by Headwaters Occupational Health and Safety. Employees will be required to complete routine tests and a review of their immunization status.

You will be required to meet with Occupational Health and Safety within your first two weeks of employment in order to review the Communicable Disease Surveillance Information Sheet you provided and to complete a Mask Fit test.

Step 4: Orientation Modules

You will be required to complete a series of mandatory orientation courses on your first day of employment, and these must be complete before you begin your new role. 

Human Resources will send you the link and login credentials for our e-learning platform after all New Hire Documentation is received and processed. If you require any assistance accessing your e-learning profile, please contact your Human Resources Representative.
