Community Paramedic Program

This innovative program is designed to make it easier for seniors, people with disabilities and those with chronic illnesses to get the care they need in their home. It is a community service launched with our partners including HHCC to be a safety net for this group of patients.

We conduct wellness checks, safe home assessments, remote patient monitoring (RPM), weight checks, blood work, urine tests and we administer medication to patients who need at-home checks after being discharged from the hospital. This program reduces visits to the Emergency Department and inpatient hospital stays. It also increases patient satisfaction with health care by reducing stress and increasing comfort and promotes a healthier community.
