At Headwaters Health Care Centre, we take great pride in the level of service that we deliver to our patients and their families. We encourage you to share your feedback, whether positive or constructive, with us. 

 

Patients and families are encouraged to communicate directly with their health care team to mutually resolve issues as they arise. Patient concerns will be forwarded to the appropriate Manager for follow up. Concerns pertaining to physicians will be forwarded to the Chief of Staff.
 

Discharge Planning

Discharge Planning assist patients and families with plans for care upon discharge from the hospital. The hospital’s Discharge Planner can provide information on:

  • community support

  • rehabilitation services

  • home care referrals

  • information and/or application to Long Term Care facilities, if required

  • medical equipment

  • respite care

  • palliative care

  • care giver relief

Hospice Dufferin

Hospice Dufferin, a non-profit community organization, provides support for individuals and families that face a terminal illness and those who are grieving.

 

The main Hospice office is located at 39 First Street in Orangeville. (open Monday to   Friday 9:00 am to 4:30 pm) A satellite office is located at Headwaters Orangeville site on F wing. Staff are available at this office Wednesday to Friday during the day.  

 

Support services are provided on a one-to-one basis by trained volunteers who work with patients and their families at either site or in the community at their home. Hospice also offers a caregiver support group, children and adult bereavement groups, education programs, and has a lending library.

 

Patients, family members, or any member of a health care team may make referrals for service. For information or referrals call 942-3313 or 941-2410 ext. 3120.


Leaving the Hospital

Your physician will determine the day of discharge. Before you leave the hospital, you will receive instructions about medications, convalescent care, and any follow-up treatment from members of your health care team.

 

Please pay any charges when you check out at the Patient Accounts Office located in the main lobby. If you required a taxi, there is a direct line that you may use located in the Emergency Department area.

Upon discharge, please stop at the Patient Accounts Office in the main lobby, next to the Information Desk, to settle your hospital account. The Patient Accounts Office is open Monday to Friday from 8:00 am to 4:00 pm. At other times, patient accounts may be paid at Switchboard.

 

You may settle your account by any of the following means:

  • Credit cards – VISA, MasterCard or American Express

  • Debit Card

  • Cash

  • Cheque

The Patient Accounts Office at ext. 2202 will handle any inquiries concerning payment of your account or hospital charges.

 

Patient Transportation

A discharge planner or nurse will meet with the patient while they are still in the hospital to discuss their discharge plan. Patients who are being discharged will be responsible for their transportation. Through Caledon Community Services, Dufferin County Community Support Services and Home at Last program residents have access to transportation services. If the patient can ride in a vehicle, friends and family can drive them home after discharge. If a patient requires transportation assistance, the hospital can provide a list of local patient transfer companies (patient transportation brochure), for which the patient will be billed directly.
