There are some items that you should make sure that you bring with you:

Your Ontario health card and 1 piece of photo ID
-
Children need to bring
their birth certificate and Ontario Health Card
-
If you have supplementary insurance coverage, please bring the
certificate, which shows the name of the insurance company
-
A credit card for
incidental expenses (toiletry packs, telephone, television rental,
etc.)
-
A list of any medications that you are taking
-
Toiletry items – such as toothbrush, toothpaste, brush/comb, shaving
equipment, shampoo, deodorant. Please do not bring scented products.

Nightgown or cotton pyjamas with a button-down top, slippers and a
housecoat
-
You may also want to bring reading material, a battery-operated
radio or laptop (wireless access is available at Orangeville site)

Ontario
residents with valid Ontario Health Card are not charged for hospital
services, except for the following:
-
Supplemental fee for a private or semi-private room (you may have
private insurance which covers this)

Telephone
service
-
Television
rental
-
Cosmetic/uninsured
procedures

Circumcision
-
Ambulance
services (you are required to pay a $45.00 co-payment for land or air
ambulance if you have a valid Ontario Health Card AND it is deemed
medically necessary by the receiving or attending physician. You are
required to pay the full $240 for land ambulance if you do not have a
valid Ontario Health Card or the ambulance transportation is not
medically necessary.)

and other supports
Canadian
residents insured in other provinces are covered in Ontario. Headwaters
will process the interprovincial claim on behalf of the patient. Canadian
residents without a provincial health card must pay for all charges.
Non-residents
of Canada are responsible for payment for all hospital services. Payment
is due at the time of service.
Medication

When
you come to the hospital please bring all the medications, or a list of
your medications, that you are currently taking. Include those you buy
without a prescription and all herbal remedies. Any allergies to
medication, or adverse reactions which you have experienced to drugs,
should be discussed with your physician. After the physician has seen your
medication, it can be sent home with your family. If your medication
cannot be sent home during your stay in the hospital, it will be kept for
you at the patient care station.
Only
medications ordered by your attending physician will be given to you
during your hospital stay. A pharmacist is available to provide you with
information on your medication.

Upon
admission and possibly during your stay, you may be asked to sign a
consent form for surgery, treatment or a diagnostic procedure. Your
physician or a member of the health care team will explain the procedure
and answer any questions you may have before you sign the consent form.
In
special cases, when you are unable to give consent, your substitute
decision-maker (specified by you at the time of registration) may give
consent on your behalf. If you have any special documents or papers
relating to your wishes, such as Power of Attorney for Personal Care, or a
Living Will, please be sure to discuss this with your health care team.

There
are three types of rooms available to patients at Headwaters Health Care
Centre:
-
Standard
Ward – more than two patients per room, covered by OHIP, no
additional charge
-
Semi-Private
– two
patients per room,
additional daily fee charged
-
Private
– one patient per room, additional daily fee charged
We
make every effort to place you in the type of room you request –
occasionally it may not be available. If this happens, we will provide a
replacement room for you, and will move you as soon as your desired room
is available.
When
you request a semi-private or private room, we’ll ask you to provide
your insurance coverage details.
Scent-free Policy

In
support of our commitment to health promotion and a healthy and safe
environment, Headwaters Health Care Centre maintains a scent free policy.
Patients, visitors, employees, medical staff and volunteers in our
hospital are requested to refrain from the use of scented perfumes,
deodorants, after-shave lotions and other scented personal hygiene
products. Scented products contain certain chemicals that trigger symptoms
in people with migraine headaches, allergies, and asthma and other
respiratory disorders - sometimes requiring emergency treatment.
When sending/bringing flowers or bouquets to patients choose low-scent
flowers. Strong scented flowers NOT allowed include freesia, lilacs,
lavenders, lilies, hyacinth and peonies.
