When you come to the Emergency Department bring:

• Your Ontario Health Card and another piece of photo ID

• Children need their birth certificate and Ontario Health Card

• Extended Health Insurance information in case you are admitted to hospital

• Written list of any medications, herbal remedies and vitamins you are taking on a regular basis

• Any other important health-related information such as allergies and past health issues
