Booking Appointments

To book an appointment call (519) 941-2410 and follow the instructions on the phone menu system to Diagnostic Imaging.  A requisition from your physician is required.  For a CT Scan a faxed requisition from your physician is required.  The fax number is (519) 941-5919.

Please bring your Requisition and Health Card with you on the day of your booked appointment.

PLEASE NOTE: Children whose parents are having an examination WILL NOT be allowed into the exam room.   PLEASE MAKE THE NECESSARY BABYSITTING ARRANGEMENTS.

Your doctor can request a copy of your x-rays to be forwarded to a specialist. Please allow 48 hours notice for the CD to be prepared. We will need to know the name and address of the physician that the CD should be forwarded to. Copies for personal use that are not requested by a physician incur a $25 administration charge.
 

Diagnostic Imaging Department
The Diagnostic Imaging department provides diagnostic services at both the Orangeville and Shelburne sites. The Diagnostic Imaging department is equipped with state-of-the-art technology. Imaging services are provided on an inpatient, outpatient and emergency basis. Emergency coverage is offered 24 hours a day, seven days a week.

In 1997, Headwaters became the first hospital in North America to have a fully digital diagnostic imaging system. All imaging examinations are performed digitally, which allows the information to be stored by a computer system. These images can be transferred electronically to many areas of the hospital, to your doctor's office or other medical institutions. All electronic transfers, and storage of health information, are performed in accordance within the Ontario Personal Health Information Protection Act.
