Kathryn MacDonald, Certified Diabetes Educator and Diabetes Educator Jean Booth of Headwaters Health Care Centre host grocery tours and pharmacy information days to help patients manage their diabetes and live well

in a non-judgmental manner that we hope helps build an ongoing, trusting relationship that enables them to care for their diabetes to the best of their abilities."

Diabetes Care Program
 

Have Diabetes but NO family doctor? A clinic is available for people with diabetes who do not have a family doctor. Call 519.941.2702 ext. 2525 to arrange an appointment.

The Diabetes Care Program offers services for adults and children:
 

§         newly diagnosed with diabetes

§         those who have been living with diabetes

§         those at risk of developing diabetes


Our Diabetes Care Team can help you and your family learn how to manage your diabetes and live well.

Health specialists on our team include:
 

§         Registered Nurses

§        Registered Dietitians

§         Physicians

§         Paediatric Social Worker


Most members of the team are Certified Diabetes Educators (CDE). A CDE has demonstrated a standard of excellence in the field of diabetes care.

All programs offer individual assessment, care and education. Group information sessions are also offered on topics of interest.

§        Self-referral is accepted

§        All services are available by appointment only

§         A physician referral is necessary for some portions of the program


Hours of Operation:
Monday to Friday: 9:00am to 12:00pm & 1:00pm to 5:00pm
The program offers some Saturday and early morning appointments.

The Diabetes Education Centre is located on the lower level of the Headwaters Orangeville site. Services are also available at Headwaters Shelburne site.
