Our membership program encourages community participation in the hospital’s ongoing development. Your support will help ensure that the necessary health services are in place to provide you with the care you need in your community, or as close to home as possible.

Benefits of Becoming a Member
· Opportunity to serve your local hospital by becoming involved and/or to join a Board Committee
· Nominating candidates for the Board
· Elect members of the Board of Directors
· Attend the hospital’s Annual General Meeting
· Vote on by-law changes
· Appoint auditors
· Receive special reports on hospital activities
· Receive advance email notice of special events
 
Board Committees
Headwaters Health Care Centre Board has four committees:
· Finance and Audit
· Governance
· Quality

As a Committee Member, you will provide oversight to ensure the hospital has effective and ongoing quality monitoring of hospital performance and improvement initiatives. Using your expertise you will provide input in developing strategies and recommending policy priorities targeting patient safety, patient care services and fiscal responsibility. 
 

Membership Fees
- Annual individual membership fee is $25 per year
- Spousal (two memberships) is $40
- Circle of Friends $0 

Membership Enrollment Criteria
· 18 years of age or older.
· Resident of Bolton, Caledon, Caledon East, Dundalk, Erin, Grand Valley, Orangeville, Shelburne and Mono, and the townships of Amaranth, East Garafraxa, East Luther Grand Valley, Melancthon and Mulmur
· Resident of the area for a minimum of three months prior.

Membership Period
Membership runs from April 1st to March 31st in the following year. Membership forms must be completed in order for candidates to participate in the voting and election process at that year’s Annual General Meeting.

Already a Headwaters Health Care Foundation Donor?
Individuals making a donation of $1,000 or greater annually to Headwaters Health Care Foundation can be enrolled as hospital members at no charge. All donor information is confidential. See the reverse for more information.
