Consent Forms

Upon admission and possibly during your stay, you may be asked to sign a consent form for surgery, treatment or a diagnostic procedure. Your physician or a member of the health care team will explain the procedure and answer any questions you may have before you sign the consent form.

In special cases, when you are unable to give consent, your substitute decision-maker (specified by you at the time of registration) may give consent on your behalf. If you have any special documents or papers relating to your wishes, such as Power of Attorney for Personal Care, or a Living Will, please be sure to discuss this with your health care team.
