Patient-Centred Care - Meeting the Needs of Our Patients

 

Headwaters Health Care Centre is guided by a philosophy of patient-centred care.  Patient-centred care focuses on the patient as the expert in his or her own care and also as the best person to define health and outcomes. Our goal is to provide the patient with the most current information to assist them in making informed decisions about their care.

 

Patient Centred Care is based on eight dimensions that have been identified through extensive research as the elements most important to patients. The eight dimensions include:         

  • Respect for patient’s individual values, preferences and needs

  • Access to care

  • Coordinating care and integrating services from all members of the health care team

  • Provision of adequate information, communication, and education

  • Enhancing physical comfort, including optimal pain management

  • Providing effective emotional support

  • Involving and supporting families and friends

  • Facilitating the transition out of the hospital through discharge planning and community support

Patient-centred care is part of our vision – Patient-centred, compassionate care in the face of change.  It’s a statement that puts the patient at the centre of what we do and builds upon our strong tradition of caring.
