Membership
This is your invitation to become a member of Headwaters Health Care
Centre
(Membership Application).
Our membership program is designed to encourage greater community
participation in the hospital’s development and to keep those with an
interest in our hospital and in health care generally informed about
hospital programs and events.
Some
of the key membership benefits include:

A
semi-annual “members only” newsletter with updates on hospital
issues and activities

The
ability to nominate candidates to the Board of Directors

Involvement
in corporate decisions and information including:

Electing
new members to the Board of Directors

Voting
on bylaw changes
-
Appointing
Auditors
-
Receiving
special reports on hospital activities
In
addition, if you wish to raise your level of participation in the
hospital’s work, you may join one of several hospital board committees,
including:
Finance
Governance
and Nominating
Planning, Partnerships & Community Engagement
Quality
Assurance/Risk Management
Individual
membership is $25.00 per year, spousal [two memberships] is $40.00 and
organization/business membership is $100.
Membership
runs from May 15th to May 14th in the following
year. Only those members who have paid their membership fee by May 14th will be able to participate in the voting and election process at the
Annual General Meeting.

Finance
Committee
This
committee reviews financial statements on a monthly basis and recommends
to the Board approval of the annual budget. The committee also receives
and reviews financial information and provides feedback on these matters.

Governance
and Nominating Committee
In
accordance with the nomination process set out in the bylaws, the
committee carries out a selection process for perspective Board members
and recommends a slate of nominees for election to the Board at the annual
meeting. This committee is also responsible for Board development,
orientation, on-going education and evaluation.

Planning,
Partnerships
and Community Engagement Committee
This
committee supports the Board by contributing to the development of the
hospital’s strategic plan and building partnerships with other health
care providers and stakeholders.

Quality
Assurance / Risk Management Committee
This
committee assists the Board in discharging its responsibility to ensure
that the hospital has effective and ongoing quality monitoring and
improvement initiatives and monitors key indicators of organizational
performance.
